Life Group Leader Reference Form

Character and Community are two of the five foundational values of The Well Worship Ministry.  Prior to involvement in platform ministry we expect applicants to build and refine character through community in life groups.  

We would like your life group leader to give a recommendation of your character by filling out the Life Group Leader Reference Form.  If you are a life group leader and are not in another life group please have a previous life group leader or a representative from the Community Leadership Team fill out your reference form.
